Your First Dental Visit

For your first appointment with us, we ask that you arrive a little early so that you can fill out our new patient paperwork. Then we’ll introduce you to one of our dental assistants and finally the dentist. After your checkup and cleaning, you should leave our dental office with fresh-feeling teeth and, depending on your needs, a treatment plan to achieve your unique smile goals.
New Patient Forms
Every new patient needs to fill out some paperwork before their first appointment. However, there’s nothing in the rule book saying that you must complete them here in our dental office waiting room. You can download, print, and prepare these forms in advance so that you’re ready to start your appointment right away!
Dental Insurance Welcome
Here at Cosmetic & Restorative Dentistry of the Keys, we are a fee for service dental office, which means that we are not in-network with any dental insurance plans or companies.
